These sound issues are really problematic. I had the severe stuttering issue with my sound. Switching from DX11 -> DX9 (as recommended in the SE thread) did solve the issue for me. You can change the setting in the Launcher window from the Gear Icon.The 44.1 Mhz sound change did not help.

Because FFXIV's sound engine is independent of the Windows Sound API, if you take out your headphones, speakers, etc. the game won't be able to fix its sound, as it's using a different device now. there's no way around it. The game detects audio device settings on startup and never again, so plugging in/unplugging headphones won't be detected properly.
